Abstract: 

Introduction & Objective: Nipple discharge is a bothering experience in half of the women, the 3th complain of the breast, and is among the signs of breast cancer. Updating the information on nipple discharge is the main purpose of this review.Materials & Methods: For this review, we chose 126 articles among 1134 articles recently published. Results: Intraductul papilloma is the main cause of nipple discharge. Diagnostic procedures have their positive and negative values.Conclusions: There is little link between nipple discharge and breast cancer. There is no definitive diagnostic procedure, but ductoscopy is promising.

Abstract: 

Objective: Management of nipple discharge by clinical finding. Materials and Methods: All of the data on patients referred for nipple discharge in period from 1990-2000 at the Department of Surgery IMAM REZA Hospital were reviewed. Results: The breast nodularity and duct ectasia was seen in 73% of patients. The age of the patient in this group was 30-40 years. Only 10 of 249 patients (5%) who presented with nipple discharge were proved to have carcinoma of the breast. Conclusion: Breast nodularity and duct ectasia is the most cause of nipple discharge (73%). Nipple discharge was not commonly associated with carcinoma and when it was the carcinoma was almost always palpable.

Abstract: 

Bloody nipple discharge in infancy has rarely been reported in medical literature. We report a 7 month-old female infant with a one month history of bilateral nipple discharge. Endocrinologic and hematological workups were within normal limits. Breast ultrasonography showed two non-echogenic cystic areas in each breast. The problem resolved after a period of observation. This is a benign condition and resolves spontaneously, but if persistent, should be properly investigated. Biopsy or excision is not indicated. We recommend breast ultrasonography for the follow-up of this condition

Abstract: 

Introduction: Nipple discharge is the third most common complaint of the breast diseases following the breast mass and the breast pain. The majority of the lesions causing pathological nipple discharge (PND) consist of non-palpable breast lesions (NPBLs). When non-operative diagnostic tools are inadequate to distinguish the diagnosis, the pathological duct should be removed to obtain definitive diagnosis. In this study, we aimed to present the results of radioguided surgery for NPBLs in patients with PND. Methods: Patients with PND who underwent radioguided occult lesion localization (ROLL) and excision in our tertiary care referral center were included in this study. Patients were evaluated in detail for age, gender, clinical data, radiological data, marking procedure, surgical results, pathological results, complication rates, local recurrence rates and follow-up period. Results: A total of 66 patients (98. 5%) were able to successfully localize the lesion with ROLL. Lesions were marked with ultrasonography (USG) in 81. 8% patients (n=54), while the rate of marking with mammography (MMG) was 18. 2% (n=12). The mean time duration of preoperative marking was 14± 4. 3 minutes. The mean operation time was determined as 35 ± 12 minutes. Malignancy was detected in 19. 7% of the patients (n=13). Surgical margin negativity was achieved in 92. 3% (n=12) of patients with malignancy. No local recurrence was detected during the follow-up period (median 62(12-116) months). Conclusion: In our study, it has been shown that radioguided occult lesion localization and excision can be easily performed technically for non-palpable breast lesions in patients with pathological nipple discharge and provides sufficient safe surgical margins with acceptable cosmetic results.

Abstract: 

مقدمه: سندرم روده تحریک پذیر (IBS) شایعترین بیماری دستگاه گوارش می باشد که در حدود 22-14 در صد افراد جامعـه را مبتــلا می نمایـد. ایـن مطالعـه با هـدف تعیین ارتبـا ط بین PND(Post nasal discharge) و IBS انجام شد. مواد و روشها: این مطالعه به صورت Case -Control انجام شد. جامعه آماری شامل کلیه بیمارانی است که در سال 1379 به درمانگاه تخصصی داخلی بیمارستان توحید سنندج مراجعه نموده اند. حجم نمونه شامل 67 بیمار مبتلا به IBS بود که سابقه اختلالات روانپزشکی و بیماریهای مزمن را نداشتند. گروه کنترل از میان بیماران غیر مبتلا به IBS و با روش Paired Matching انتخاب شدند.نتایج: براساس نتایج این مطالعه 59.7 در صـد مبتلایان به IBS,( چهل نفر) را زنان و 40.3 در صد (27 نفر) را مردان تشکیل مـی دادند. میانگیـن سنـی ایـن بیـماران 27.9 سال با انحراف معیـار 8.73 سال بود. همچنین 41.8 درصـد 28) مورد) آنها در رادیـوگرافی از سینوسهای پارانازال شواهـدی دال بـر سینـوزیت داشتـند. بین سابقه سینـوزیـت و IBS رابـطه معنـی داری وجـود دارد P=0.01) و OR=4.27 و CI=1.19-16.74 و X2=5.37 و df=1) . همچنین رابطه بین PND و IBS معنی دار می باشد P=0.001) و CI=2.29-13.99 و OR=5.6 و (X2=18.69.نتیجه گیری: براساس نتایج این مطالعه عفونتهای دستگاه تنفسی فوقانی بویژه سینوزیت با IBS همراهی دارد و مطرح کننده این موضوع است که شاید IBS یک اختلال منتشر است که در آن سیستمهای مختلف بدن درگیر می باشند.

Abstract: 

Bloody nipple discharge is among the most common problem in breast surgery. This is because the benign intraducatal papilloma is the leading cause of bloody nipple discharge, although in malignant carcinoma of the breast any discharge is bloody.This survey was carried in order to determine ranking rate of causes and coincidence of intraductal papilloma, bloody nipple discharge and breast mass, for avoidance of exaggerated or insufficiently appreciated treatment (unnecessary mastectomy versus insufficient local operation) in these patients. This study performed retrospectively by chart review of 1218 patients with any impression of breast problems, who enconsecutively and admitted in Hazrat Rasul and Imam Khomeini University Hospitals during 10 years (1986-1995). Among these patients 28 cases with benign papilloma lesions were determined and classified to solitary intraductal papilloma (SIP, 15 cases), multiple intraductal papilloma (MIP, 9 cases) and papillomatosis (4 cases).The mean age of patients with SIP were 40.2 years and in MIP were 39.5 years. Nipple discharges were detected in 80% and palpable mass was also detected in 73.3% of patients with SIP. In patients with SIP 88.3% of discharges were bloody. In 53.3% of patients with SIP discharges and breast mass were detected coincidentally. In patients with MIP palpable mass was determined in 88.9% and discharge 66.7%. Carcinoma of the breast was not found in these patients.There were no significant relation between the rate of palpable breast mass, malignancy of lesions and nipple discharges in these patients.
